The usual dose is 20 mg twice a day (every 12 hours), intravenously.
Zollinger-Ellison syndrome:

During general anesthesia to prevent aspiration of gastric juice:
Before general anesthesia, 20 mg of the drug is administered intravenously on the morning of the operation or at least 2 hours before the operation.
A single intravenous dose should not exceed 20 mg. When administered intravenously, the injection powder must be diluted in 5 ml of 0.9% sodium chloride (solvent ampoule) and administered slowly (over 2 minutes). The drug administration time should not be less than two minutes! When used in infusion, solution
must be administered over 15-30 minutes.
